Insurance and Rates
In-Network Health Insurance
If you have In-Network health insurance, we will be happy to check the details of your benefits before your first visit. We will do our best to provide you with your deductible and/or co-payment amount in advance. As health insurance companies remind you, coverage quoted is not a guarantee of payment or verifies eligibility. Payment of benefits are subject to all terms, conditions, limitation, and exclusions. You are ultimately responsible for all fees, if your insurance does not pay.
What does this disclaimer from your insurance company mean for you?
It means the following:
I am responsible for verifying my benefits, especially if they do not match the information provided by my insurance in my handbook or member service portal.
Benefits sent to me by Counseling Services of Portland are quoted by my insurance company.
Counseling Services of Portland is not responsible for any information that is not quoted correctly or left out by my insurance company.
If my insurance denies payment for service, I will be responsible for payment.
Most of our therapists are In-Network with:
Kaiser
MODA
OHP Trillium
Pacific Source (some plans)
Out-of-Network Benefits
We do not bill Out-of-Network insurance. If you are interested paying our cash rates for your out-of-network insurance benefits and need a superbill, we can provide those upon request to our Lead Billing Specialist.
Private Pay Rates
The rates for private “out-of-pocket” pay are listed below for those who are not using their insurance or do not have insurance. Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your health care will cost.
Under the law, health care providers need to give patients who don’t have certain types of health care coverage or who are not using certain types of health care coverage an estimate of their bill for health care items and services before those items or services are provided.
You have the right to receive a Good Faith Estimate for the total expected cost of any health care items or services upon request or when scheduling such items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
If you schedule a health care item or service at least 3 business days in advance, make sure your healthcare provider or facility gives you a Good Faith Estimate in writing within 1 business day after scheduling. If you schedule a health care item or service at least 10 business days in advance, make sure your healthcare provider or facility gives you a Good Faith Estimate in writing within 3 business days after scheduling. You can also ask any health care provider or facility for a Good Faith Estimate before you schedule an item or service. If you do, make sure the health care provider or facility gives you a Good Faith Estimate in writing within 3 business days after you ask.
If you receive a bill that is at least $400 more for any provider or facility than your Good Faith Estimate from that provider or facility, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate and the bill.
